The license for a pharmacy

Pharmacy - health care institution, whose main task is provision of population, health institutions, enterprises, institutions and organizations of drugs through retail outlets;

Pharmacy - structural unit of the pharmacy, which is created in health care facilities for the trade in drugs;

Pharmacy - a structural subdivision of Pharmacy, which is created in enterprises, institutions and organizations for the retail trade in medicines sold without a prescription;

Basic Conditions of retail of drugs:

- Availability of a lease agreement or document to the ownership of the pharmacy, which should be placed in a permanent structure with the trading room and take no less than 18 sq.m. (In health care settings may be set up pharmacy items area not less than 8 square meters);

- Drugs of various categories should be stored separately (poisonous, narcotic);

- Finishing materials pharmacy premises must comply with the recommendations MOZU;

- Availability of standard technical and legal documentation;

- Required the presence of drugs for pre-hospital care;

- Personnel must have special pharmaceutical education and qualifications (pharmacist, pharmacist). Order of the enterprise is appointed head of the pharmacy (drug stores). All workers should have health books.

Necessary documents for obtaining a license:

- An application;

- Notarized copy of state registration of a business entity;

- Notarized copy of certificate of registration in EDRPOU;

- Notarized copy of passport of drug stores (pharmacies, the structural unit), compiled on a form approved by the Cabinet of Ministers of Ukraine;

- Power of Attorney.
